SLS

Supported Living Services (SLS) consist of a broad range of services to adults with developmental disabilities who, through the Individual Program Plan (IPP) process, choose to live in homes they themselves own or lease in the community.

    Maintaining a clean, safe, and organized living space

  • Money Management
    Handling everyday finances including budgeting, bill payments, shopping, and banking

    Supporting daily hygiene routines and promoting overall well-being

  • Medical & Dental Appointments
    Scheduling, attending, and following through on healthcare visits

  • Medication Management Support
    Assisting with reminders, organization, and understanding prescriptions

  • Self-Advocacy & Communication Skills
    Encouraging independence, personal rights, and decision-making

  • Public Transportation Use
    Navigating bus/train routes, using ride services, and developing travel independence

  • Housing Support
    Finding and maintaining safe, affordable housing and choosing compatible roommates

  • Access to Public Benefits
    Assistance with Medi-Cal, Medicare, SSI, Social Security, and other benefit programs

  • Community Safety & Awareness
    Understanding how and when to contact emergency services like fire, police, or medical help

  • Recreation & Social Participation
    Engaging in hobbies, fitness, and natural outdoor environments to enhance social well-being
